|
FamilyCare offers health
care coverage to parents living with their children 18 years old or younger.
FamilyCare also covers relatives who are caring for children in place of

| Illinois Department of Public Aid/Medicaid Assistance |
Medicaid is a jointly-funded state and federal government program that pays for medically
necessary services. Medicaid pays for medical services for children and
their caretakers, pregnant women, and persons who are disabled, blind or 65
years of age or older.

Illinois Veterans' Homes |
The State of Illinois operates four veterans homes located
throughout the state. Quincy, Anna, Manteno and LaSalle all offer
skilled care with special needs units
while the Quincy and Anna Homes also provide domiciliary care, an
assisted living environment where a veteran and
their spouse can reside.

Illinois Rx Buying Club

A program that offers all prescription drugs approved
by the FDA that are available through participating pharmacies and the program's mail-order option at costs
up to 30% of their normal costs.

Developed by the State of Illinois, and open
to all Illinois and Wisconsin residents, the new I-SaveRx program offers a
safe, simple and affordable way to purchase the medication refills you need
- at savings of up to 50 percent.

|
A state program that
offers health care coverage to children, pregnant women and their babies or
helps in paying premiums of employer or private health insurance plans.

| SeniorCare Illinois
|
A rebate program available for seniors that
currently have pharmaceutical coverage.

| TRICARE
|
TRICARE is the Department
of Defense's worldwide health care program for active duty and retired uniformed
services members and their families. TRICARE consists of TRICARE Prime, a
managed care option; TRICARE Extra, a preferred provider option; and
TRICARE Standard, a fee-for-service option. TRICARE For Life is also
available for Medicare-eligible beneficiaries age 65 and over (effective
Oct. 1, 2001).

United Concordia |
The TDP is a voluntary,
comprehensive dental program offered worldwide by the Department of Defense
to family members of all active duty Uniformed Service personnel and to
Selected Reserve and Individual Ready Reserve (IRR) members and/or their
family members. United Concordia is the administrator of this program.
